Drawing on his extensive experience in Singapore’s healthcare system, A/P Low will discuss how population health approaches can be translated from research into real-world impact. He will examine the challenges of scaling successful pilot programmes into system-wide transformation and explain why the shift from healthcare to health is an operational imperative with global relevance.

Seminar Abstract
Singapore’s healthcare system has long been recognised for its efficiency and clinical excellence. Yet as the nation confronts a rapidly ageing population and a rising burden of chronic disease, the limitations of a purely episodic, treatment-focused model have become increasingly apparent. This talk examines Singapore’s ongoing transformation from a system centred on treating illness to one oriented around sustaining health and enabling healthy longevity, supporting the population not merely to live longer, but to live well, with preserved function, independence, and quality of life in their later years.
Drawing on the work of the SingHealth Regional Health System, Associate Professor Low Lian Leng will explore how population health approaches are being translated from research into real-world impact. This includes the design and evaluation of integrated care programmes that bridge primary, community, and acute care, and the use of data, implementation science, and community partnerships to identify at-risk populations, close care gaps, and deliver interventions that are both effective and contextually appropriate.
A/Prof Low will also reflect on the challenges of scaling what works, moving from pilots to system-wide change, and argue that the shift from healthcare to health, with healthy longevity as a north star, is not merely a policy aspiration but an operational imperative, with lessons relevant to health systems around the world.
About the Speaker
Assoc Prof. Low Lian Leng is currently Chairman and Senior Consultant, Population Health and Integrated Care in Singapore General Hospital, and Director, SingHealth Centre for Population Health Research and Implementation. He is also faculty at Duke-NUS Medical School, Singapore University of Technology and Design, and Singapore University of Social Sciences.
As Chairman, Population health and Integrated Care in SGH, A/P. Low has spearheaded innovative models of care such as Community Nursing, Hospital at Home and Integrated Senior Health and Wellness Hubs. He also has a deep interest in health services research, especially in the areas of population health, innovative integrated care delivery models, value-based care and data analytics. A/P. Low is also the principal investigator of the Community Ageing in Place Ecosystem project funded by the Ministry of Health Singapore, that integrates the physical built environment, climate adaptation strategies, technology and health-social care integration for ageing well in Singapore. A/P. Low is supervising post docs, PhD students, data analysts, research fellows and associates in their professional work.
To date, A/P. Low had secured close to $16M SGD in competitive research funding as PI or Co-PI from grant agencies such as the National Medical Research Council Singapore, Artificial Intelligence Singapore, Novartis Foundation, National Health Innovation Centre Singapore and the Ministry of Health Singapore. He had published more than 150 articles, with a h-index of 41 as of June 2026.
About the Moderator
Prof Eliza Wong is a Registered Nurse of Canada and Hong Kong (HK). She further pursed her PhD at The Chinese University of Hong Kong with focus on the implication of Chinese medicine in public health. Also, she is an Accredited Mediator of HK International Arbitration Centre and HK Mediation Accreditation Association Limited.
